This is a single module that may be taken in isolation and is an alternative compulsory for students taking the Renal Practice short course. The module provides the opportunity for students to critically study the needs of individuals with long-term conditions encompassing specialist areas such as renal, neurosciences, gastroenterology, respiratory, cardiology and diabetes.
Students are expected to develop a greater understanding of the bio-physiological changes that affect bodily systems that lead to specific long-term conditions. A blended learning approach is used within the module that includes key work packages that may be completed by the students at home (over two days), skills sessions developing students assessment skills of key systems within the body and a mixture of seminars and lectures. On completion of this module, students will have be given the opportunity to develop their clinical assessment skills of patients with a variety of key long-term conditions such as Cardiac Failure, Stroke, Chronic Renal Failure, Diabetes, Chronic Obstructive Pulmonary Disease and Gastrointestinal Disease. Students will also have an opportunity to critically analyse prevalent issues linked to long-term illness such as concordance, mental health issues, health promotion and patient education.
The module assessment comprises:
A poster presentation including oral presentation.

Access to the Clinical Management of Long Term Conditions short course is open to practitioners who meet the following criteria. You must:
- Be a UK registered health or social care professional with a minimum of 12 months professional practice.
- Have the support of the manager of your practice area.
- Be able to produce evidence of the ability to study at your proposed level of entry if requested.
- Be successful in the selection process for this module/course (interview or other).
- Be employed or have access to a health care setting that will enable achievement of the learning outcomes.
